Esoteric Acupuncture Foundations Seminar

I have volunteered to participate in a paid seminar with or organized by Frank Turano-Cutler, L.Ac., and Healing Path Acupuncture, LLC to include, but not limited to, any and all services provided, such as energy healing, distance energy healing, intuitive coaching, live or recorded teleconferences, webinars, classes or seminars.  

I recognize that Frank Turano-Cutler, L.Ac.is a licensed acupuncturist and any medical advice given will be in terms of Chinese Medicine. I understand that Frank Turano-Cutler, L.Ac. and Healing Path Acupuncture, LLC technology and personnel do not diagnose, treat, prescribe or claim to cure any disease or ailments. This seminar is not intended to replace licensed medical diagnosis or treatment in any way. Frank Turano-Cutler, L.Ac. and Healing Path Acupuncture, LLC strongly advises that you seek professional advice as appropriate before making any health decision. I realize it is my responsibility to secure such advice.

I acknowledge and agree that I assume the risks associated with any and all activities and/or programs in which I participate.

I acknowledge and agree that no warranties or representations have been made to me regarding the results I will achieve from any insight or program. I understand that results are individual and may vary. 

I acknowledge and agree that Frank Turano-Cutler, L.Ac.reserves the right to cancel scheduled seminars at his discretion.  I further acknowledge and agree that Frank Turano-Cutler, L.Ac., is not financially responsible for travel or other costs incurred by me in the event of a cancellation, except for the refund of all deposits, tuition and fees.  Frank Turano-Cutler, L.Ac. highly recommends that all participants consider travel insurance to cover costs resulting from unforeseen cancellations. 

I acknowledge that I have thoroughly read this waiver and release, and fully understand that it is a release of liability. By signing this document or agreeing to the terms of service by checking the box provided during the purchasing process, I am waiving any right I or my successors might have to bring a legal action or assert a claim against Frank Turano-Cutler, L.Ac.and Healing Path Acupuncture, LLC and agree to hold harmless its principles, directors, officers, employees, agents, contractors, consultants, volunteers and others associated with Frank Turano-Cutler, L.Ac., and Healing Path Acupuncture, LLC and their heirs, and agree to hold harmless from any claim or liability whatsoever and for any damage or injury, personal, emotional, psychological, financial, or otherwise, which I may incur arising at any time out of my participation, purchase or subsequent application of the information, ideas, suggestions, techniques, and other materials contained herein.  

If any court of law rules that any part of the Waiver and Release is invalid, the Waiver and Release stands as if those parts were struck out.

This waiver and release form is to be construed under the laws of the State of Colorado; any proceedings to interpret or enforce this agreement must be brought in the Routt County Courts of the State of Colorado.
